V. What does MEDITRUST-USA Cover?

a.) All doctor visits minus $10 copay.

         b.) All in hospital care minus $300 copay.

         c.) All ER visits minus $20 copay.

         d.) All lab and x-ray expenses less $15 copay.

         e.) All prescription drug charges minus $3 copay.

         f.) All eye care less $20 copay.

         g.) All medical related supplies less $20 copay.

Copays will be waived for those individuals who are not employed and have no other source of income, such as pensions or trust/annuity payments.
